Fees & Insurance

The fee for an initial evaluation will vary depending on the complexity of your problem. Other services such as diagnostic x-rays, MRI, casts, splints, injections, etc., involve additional fees.

All co-pays and co-insurance payments will be required at the time of service as directed by your health insurance policy.

If you have not met your insurance deductible, if you have no insurance, or if a third party is responsible for your bill, we require payment at the time of service. Alternate payment arrangements can be discussed with the billing department.

We accept VISA, MasterCard, American Express, debit cards, checks and cash. Returned checks will have an additional fee for processing.

Remember that insurance contracts are between you and your insurance company. Insurance policies often do not provide full payment of medical costs. Payment of your account is your responsibility.

As a service to our patients we file insurance claims directly to the carrier. Our office accepts assignment on Medicare claims. Workers’ compensation and automobile accident claims must be pre-approved.

If you require surgery or other diagnostic testing, please check your insurance policy to determine if authorization or second opinion is required.

If you require completion of disability or FMLA forms due to your orthopaedic condition, please mail or bring them with you at the time of your appointment. Please allow seven to ten business days for completion of these forms. In consideration of all of our patients, these forms are completed in the order in which they are received.

Some of these forms will have a processing fee.

If you require your records to be sent from our office to another facility, OR from another facility to our office, please download these forms and send to the appropriate facility.
